Combination therapy outperformed monotherapy
Men taking low-dose sildenafil + low-dose tadalafil scored higher on the International Index of Erectile Function (IIEF) than those taking full-dose sildenafil or full-dose tadalafil alone.
No increase in adverse events
The combination at lower individual doses produced fewer total side effects than full-dose monotherapy. This challenges the assumption that combining PDE5 inhibitors increases risk.
Dual pharmacokinetic benefit
Sildenafil provided rapid onset (15-30 minutes) while tadalafil provided extended duration (up to 36 hours). Patients got the best of both drugs' timing profiles.
Patient satisfaction scores were highest in the combination group
Beyond clinical measures, patients in the combination group reported the highest satisfaction with spontaneity and reliability.
What this means for you
This study validated what pharmacology would predict: two drugs hitting the same target (PDE5) but with different timing profiles complement each other. You get the fast onset of sildenafil (reliable for planned encounters) and the long window of tadalafil (reliable for spontaneous ones). And because each drug is used at a lower dose, side effects actually decrease.
Limitations
The study was 8 weeks, which captures short-term efficacy but not long-term safety of the combination. The 180-patient sample is modest. The study was not blinded, which may introduce bias in patient-reported outcomes. Regulatory bodies have not formally approved the combination as a single product.
